What To Bring To An Appointment

Before your first appointment with one of our physicians, we would like to have you complete the medical history and patient demographic forms. We also request that you obtain medical records (including X-rays, CT or MRI scans, etc.) from your referring health care providers and bring them with you if possible.

Please bring a list of all medications you currently take, including over-the-counter and herbal supplements. Include the exact name of medication, dose of medication, and how often you take it. If it is more convenient, you may bring your medication in its original container.

Bring your current insurance card.

Bring your completed forms (which can be downloaded). This saves time – yours and ours.

Co-Payments

Co-payments are required to be paid at the time of service. We accept cash, check and all major credit cards.

Cancel An Appointment

In the event you cannot keep your office appointment, please contact our office at least 24 hours in advance. You may leave a message if the office is closed. If you are scheduled for an in-house procedure, it is necessary to contact our office 48 hours in advance.

Prescriptions

Call for prescription refills during office hours, 8:30 a.m. – 4:30 p.m., Monday through Friday or contact your pharmacy for the refill. Call well before you have run out, since some prescriptions may not be able to be filled on the same day.

Medical Records

We handle all medical records with strictest confidence. We require your written permission and signature before we can send information to anyone. Please allow two weeks for completing the paperwork.
